We are all used to ethics discussions centred on AI problems and the behaviour of autonomous cars, however, it’s possible that the most damaging IT ethical questions and system train-wrecks are more mundane.

In July 2016, the Government introduced the Online Compliance Intervention to recover social welfare over payments.Robodebt (as it became known) was withdrawn in May 2020. About 470,000 people were effected and between $700M and $1.2 B needs to be repaid
