Be complete and accurate
Complete all sections. If any section is not applicable to you, write “N/A” (“Not applicable”). If your application is incomplete it may be returned to you and this will delay the processing. Do not leave any blanks.
Note: If you need more space for any section, use an extra sheet of paper and indicate the number of the section you are completing and submit it along with your form.
